In a nation first, the WA Department of Health partnered with WA based Aboriginal Community Controlled Health Services (ACCHS) to deliver locally led, place-based, culturally responsive aged care services through the ACCHS TCP Pilot. The TCP Pilot ACCHS place a strong focus on re-forming relationships or connecting clients to community to empower clients to take care of their health. Pilot ACCHS use the opportunity to expand the types of services they deliver to better support Aboriginal people in their region and identify gaps and cultural barriers for local Aboriginal people accessing aged care, contributing to a national evidence base on improving accessibility of aged care services for Aboriginal people.
