The State Party requires the delivery of comprehensive community-based primary health care that is easy to access and tailored to people experiencing or at risk of homelessness. The Service must provide low-barrier, easy to access primary healthcare services consistent with a “no wrong door” approach: they are free (or low cost), conveniently and consistently located, welcoming, and trauma informed. The Service must be designed to address multiple needs in one location or visit, with clear pathways into housing and ongoing supports where required.
