Pathways Home is further strengthened by dedicated Disability Housing Advisors based in the five partner agencies supporting the initiative: Department of Housing and Public Works (Specialist Response Team) Department of Families, Seniors, Disability Services and Child Safety Department of Youth Justice and Victim Support Queensland Health Queensland Corrective Services Pathways Home will provide specialist and inclusive supports, information resources, referral and capacity building activities to directly and indirectly uplift people with disability achieving safe, stable, and suitable housing outcomes. Pathways Home will work with the social, community and housing service sectors to facilitate coordinated and collaborative, multi-agency and person-centred approaches to source diverse housing solutions for people with disability and supporters. Critical to Pathways Home operations is building networks, partnerships and collaborative processes with a range of services and providers, both in the local area and regionally. Visiting organisations will be invited to provide information, advice and skill-building workshops.
