Summary and scope
This Invitation to Offer is for the establishment of eight (8) Neuro Wellness Hubs (Hubs), providing a coordinated model of care for Queenslanders experiencing neurological health conditions, and the delivery of the neuro community navigator program.The Neuro Wellness Hubs will provide a community-based space to deliver neuro wellness services, resources and information to people with neurological conditions (and their families/support network), and complement the services provided by acute speciality teams, general practice and other primary care services.The neuro community navigators’ program will support system navigation by connecting people with neurological conditions to broader systems including (but not limited to) the National Disability Insurance Scheme (NDIS), My Aged Care, Medicare and mental health services.
